GLP-1 Drugs and Lean Mass: What the Evidence Shows
Lean mass can change during GLP-1-based weight loss, but the size and meaning of that change depend on the drug, population, method, and study.

Total weight does not describe body composition
GLP-1-based medicines can produce substantial weight loss, but a scale does not separate body fat from lean tissue, water, glycogen, bone, and other compartments. The useful research question is how much each compartment changed, how it was measured, and how the change compares with total weight lost.
What the evidence reviews found
A 2024 systematic review and meta-analysis covering 22 randomized trials and 2,258 participants found reductions in body weight, fat mass, and lean mass with GLP-1 receptor agonists. Across that pooled evidence, lean mass accounted for approximately one quarter of total weight lost. This was an average across different drugs, populations, durations, and measurement methods, not a forecast for an individual.
A separate systematic review of semaglutide included six studies and 1,541 adults without diabetes. Reported lean-mass change ranged from nearly none in some studies to as much as 40 percent of total weight lost in others. That variation argues against using one percentage as a universal rule.
Why estimates vary
Studies use different body-composition methods, including DXA, bioelectrical impedance, and imaging. Hydration, glycogen, timing, age, sex, diabetes status, baseline body size, treatment duration, and total weight change can all affect the estimate. Lean mass is also broader than contractile skeletal muscle and does not measure strength directly.
The evidence supports monitoring body composition as a distinct question. It does not support diagnosing a person from a scale result or assuming that every GLP-1-based medicine has the same effect. Treatment, nutrition, exercise, symptoms, and monitoring belong with a qualified clinician who can evaluate the full medical context.
